Ok, i changed CGI: 'xxx' to CGI: 'sa1" .
Object.extend(FoxInCloud, {
CGI: 'sa1',

the form behavior is the unchanged i tried CGI: 'sa' since 'sa' is the FAA code. still no change.
i made form2 modal - now form 2 comes up.
