Looks like your server does not answer the form initialization request.
What happens if you hit F5? does the server send back page HTML?
This is what i can see:
Can you see something happening in firebug?
e.g.
console panel
network panel
When i run my app and i call it from the browser the main form is loaded but the browser continues to show the hourglass like it have still something to load and nothing happens.....In Fox i have no errors....the app in lan mode works fine...
